Our Services

  • Assesses the patient’s physical condition and physical therapy plan in order to help the patient with their activities, help alleviate and prevent pain and reach safe ambulation, transfer, and balance gait.

  • Works in conjunction with other services for patient to adopt function in activities of daily living.

  • Assesses patient’s speech problem and will improve your communication ability.

  • Assesses your psychosocial needs and long term range planning to adjust in the community.

  • Assists the physician and other agency personnel in evaluating the dietary needs of patient.

  • Assists you with patient’s activities of daily living such as grooming, bathing, hygiene, eating and light house keeping, which is supervised as necessary.

  • We assess the patient’s need for equipment such as cane, walker, wheelchair, etc. and provide the patient with the highest quality equipment through our DME contractor.
